Custom form to route requests based on input criteria

Hi, I am looking for some advice and direction on how to use a custom form to route requests to various mailboxes based on what is input into the form. I do have some experience with VBA in Excel macros. I'm hoping to find a pretty good template I can modify.

I don't have any templates like that, that I can recall, but that doesn't mean there isn't one floating around. Is this mail template or calendar, or contacts?

Does it need to be a form? A macro might be able it do.
