Textarea and Radio Buttons in Outlook Email

Hi All,
I want to design a Outlook Feedback email with Radio Buttons and Textarea . It is not working in Outlook 2007. So How can I design My Feedback Form with all my requirements.

Are you sending it to other outlook users in your organization or to people on the internet? Outlook doesn't support active content - you can try making it in an HTML editor and inserting it into the message (Use Insert file, As text and select the HTML page you created).

for best results and a solution that works for everyone, use a web form and send a link to it.
