Outlook Forms???

Hi... a number of years ago I designed and published a form that was used to track work... It had some fileds that automatically generated date and time stamps when people adde to the "Message area.... Now I cant remember how i did all this.... Im not even sure if I designed it from outmlook or via Infopath.....

The crux is i need to have a form with the ability for it to be frowarded on.... and each time a new comment is made in a specific filed the I need an auot generated date and time stamp... Does any one have nay ideas??
