    When I was running Outlook 2003 under XP, there was a Choose Form option on the toolbar. Now that I'm running it under Win 7, there isn't. So instead of clicking on it directly, I have to click File, then New, then Choose Form. How do I get it back on the toolbar? While I'm at it...
