I'm puzzling over this. Nothing really jumps out at me, but I do have the following thoughts:
1. Are the added appointments actually making it onto the calendar? In other words, can you see them via OWA or some other non-cached source?
2. Do you have an unusual Full Access or Send As...
Here's an update on this problem.
For whatever reason -- and I am sure there's something different here -- removing the Hotmail account did _not_ result in the From field de-populating.
In the end, I wound up:
1. Creating a new Outlook profile.
2. Attaching Hotmail to that profile...
Interesting. When I get in front of the workstation I'll perform a couple of tests. Maybe I didn't look closely enough when I removed the Hotmail account. I'd do it myself now but the user has Outlook 2007 and I only have copies of 2010.
