Outlook forms text input

Exchange Server
I am new to outlook forms I have no idea where to start but I been asked to see if I can create a form that has
Sale Amount:

Loan Amount:

Closing Date:

Address w/County:

Primary:

And other fields but when I design a basic form with this information and input any text, email it to myself it is blank. Ive tried to google this some but I can't seem to find any reference material for text box forms
