Using Follup in Custom Forms Outlook 2013

I created a new form in Outlook 2013 People, Everything works ok, except that when I go to add a Follow up and reminder, the drop down window appears but when I select a time for the reminder, nothing happens. Is there something I need to add to the form to have this work? thanks, don
 
Hi Thanks, Yes follow up works in the form that is original default for Outlook, I want to change that form to reflect my business needs, we do life insurance so use home addresses etc, spouse info, birth dates etc. I am using Outlook for Sales lead follow up. I also noticed when I assign a category to the form it does not show up either, on original form it show up as a stripe across the top. Maybe I can make a video to send to show you better. thanks, don
 
Hi I tried to upload video but the file was too large so will try to send some screen shots. Can you please tell me how the categories and follow ups are implemented in Outlook people? Is there some reason they will not work in a form? thanks, don
 
Hi, Here are 3 pics, the first one shows the category blue ribbon and reminder from follow up flag in upper right corner. then next pic shows the new form I set up, The follow up flag drop down window will come up, but if I set a category or reminder they do not show as in the first slide. I am not sure how these are implemented in the original Outlook forms. Please let me know if you can figure this out. thanks, don

There is a category control in the control toolbox. Right click on an empty area in the control toolbox and chose the option to show more controls. There is a category control in the list. Add it to the toolbox and then to the form.

(ill look at the screenshots again when I'm on my big computer.)

Hi Diane, Thanks you're the best!. The category stripe now works, Would you please look at the Follow up flag not posting? Is there maybe control box item for it too? don
 
Hi Diane, Were you able to look at the follow up issue I am having on the form yet? Basically it was similar to the category issue where it does not work on the new form. I get the drop down window to select follow up but no display of follow up in top of form. Thank you , don
