Importing csv file to custom form/fields?

Can I import a csv file to a custom form/fields? I'm lost. My mapping wizard does not show the form and custom fields that I want to map/import to. Is this possible?
 
I have about 24 custom fields I created and used the Developer to design my form starting from the standard 2013 outlook contact form. When using the import wizard, csv file, destination folder, I do not see anyway to create a new folder under Contacts, if that's indeed where my customized form should be.
I have tried to save/publish my custom form into all the Folders...Outlook Folders, Personal Forms Library,
Contacts, no luck showing up in the mapping wizard. I'd appreciate any suggestions from someone more knowledgeable. Thanks Larry Corban University
 
The mapping wizard only support the default fields - you map your 'custom' names to the default fields.

See http://www.outlookcode.com/d/code/convertfields.htm for one method - I'm not sure it's the best method these days, but I don't have time to check for better methods.
 
Actually in looking at that code again, I think I'd add the field and values to the folder then set the form. Read the CSV values into a string then create the field and set the value. There is a chance it won't work, since I haven't tested it for this purpose. :( (But I've used it to create custom fields using data from other parts of a message or item.)
