Outlook 2010 Custom Form Problem

Good Day All,

I am having an issue with implementing a Outlook form. I am new to form creation and I think I am missing something. I fill out a similar form longhand and have created this form to cut time. This will only need to be distributed inner office. Everytime I send this via email it comes up as a blank page. Any help would be awesome. I have attached a screenshot of the form.

If this is a coding issue please direct me to where I can grab the snippet of code to fix the problem.

I'd suggest the MSDN forum might be better - Outlook for Developers Forum - we can handle simple questions here but I don't do much forms work. I think you either need to publish it in the org forms library or send the form definitions with it. (Or publish it in each users mailbox.) The MSDN experts can tell you exactly.
