Saving Published Outlook Form as msg

I have created a customised Outlook Form using Bound Fields with Drop Downs and open text Fields. When the Form is completed and the user attempts to save the form as a *msg, all the fields and contents disappear when opening the saved message. I've also tried saving as an *otf, but I get the same result.

Is there a way of getting the populated form to save as you see it on screen?

Thanks for any help offered.
 
Did you create separate read and compose pages? You need the fields on both pages. Look at the Page button to see what is selected.
