Outlook 2010 form is displaying old form layout

I copied an Outlook form named maintenance-2005 and renamed it to maintenance-2014. I modified this form in Outlook developer and saved it to my local C drive. When I open the new form, fill in the information and click Send, the old version of the form is being sent. I've opened the email from my Sent Items and it contains the old version of the form. What am I missing?

Are you using version #? if you don't, you can end up with one-offed forms and inconsistent results. In Forms Designer, look on the Properties tab - always use the version # and increment every time you publish it.
 
I think the part I am missing is the publishing to MS Exchange. Do I have to publish this form to Exchange? When I choose the form, I have it saved in "User templates in file system". Should I be saving it in Organizational Forms Library? I don't see the previous form (maintenance-2005) in this library.
 
If other people need the form, either to send or receive, then it should be in the org library.

Your forms cache is probably corrupt - find it and delete it. It's in %localappdata%\Microsoft\forms
