Custom Outlook form_Validations in combo box

Hi,

I have a problem if you can solve that.

I need to create a form for the user (screenshot attached). This form will have combo boxes and text boxes.

Now the problem is values in combo boxes will be shown on the basis of some logics. For example, suppose in product line subtype we have 3 values (Red, Green, Yellow). Now if user selects Red from this box, then basis of this value Section code box will have the drop-downs values. For example, Apple, Blood, Sun. Now if user selects Apple for this box, then Profit code will have different values.

Now the user does this task every single day and multiple time in a day. So I need to create a form, so that the user selects that form from outlook, fill in the information and sends.

Can you please help me with this.
