Custom Outlook Form - Populate Information from Radio Button / Check Box

My contacts pay for courses, I created a radio button for payment type (cc, check, cash) and check boxes for course titles.

Radio button - How can I populate the information from the radio button into the user defined field - paymenttype?

Courses - what is the most efficient way to see what courses the user is taking. Since it could be multiple check boxes for multiple courses, does each course need a separate user defined field or can I just have one with the courses separated for example by semi colon. Again - how would I populate the user defined field from the information of the check box? As a second thought - would it be better to use a combo box for the courses rather then check boxes?
