2nd meeting confirmation notification ending up in Deleted Items if attachment is added

I have a user that is on Outlook 2019 32bit. This is only happening with one employee.
- When they get a meeting notification the first time it comes into their Inbox like you would expect.

- If the recipient accepts the meeting invite then the next step with attachments ends up in the Deleted Items folder.

- If the meeting organizer then realizes they forgot an attachment and updates the meeting with that attachment then that Email notification goes directly into the attendees Deleted Items folder.

- If the meeting organizer updates the meeting time then everything works correctly and the attendant gets the Email update in their Inbox.

I have tried everything I can find on Google searches and nothing seems to remedy this problem so I am hoping someone has run across this or familiar with why it happens.

That smells like something client-side is “helpfully” processing meeting updates, not normal Exchange behavior. First thing I’d check is Outlook rules, including hidden/client-only rules, and whether “automatically process meeting requests and responses” or any add-in is touching calendar items. Start Outlook in safe mode with outlook.exe /safe and test with an attachment update. If it behaves, disable add-ins one at a time. Also check Deleted Items for the message properties if possible, it may show whether a rule moved it. Since it’s only one user, I’d also try a new Outlook profile before chasing server settings too hard. Outlook profiles do occasionally go feral.

Thank You for the ideas on where to look and I will check them out and see if it provides any solutions. I appreciate the input.

Spot-on advice, as this definitely sounds like client-side auto-processing or an aggressive rule going rogue on meeting updates. Another setting worth double-checking in Outlook Options under Tracking is whether "Update tracking information, and then delete responses that don't contain comments" is enabled, as that can sometimes aggressively route items straight to Deleted Items. Rebuilding the local Outlook profile is usually the fastest fix once you eliminate safe mode and add-in culprits.

That’s a really interesting one, especially since it’s isolated to a single user and time changes are delivered normally. I’d suspect something specific to that mailbox/profile rather than Outlook’s meeting processing in general. Have you tried recreating the Outlook profile or testing the same mailbox in OWA? I’d also check the user’s Inbox/Deleted Items rules and any client-side add-ins, particularly anything that handles calendar or attachments.
