2nd meeting confirmation notification ending up in Deleted Items if attachment is added

I have a user that is on Outlook 2019 32bit. This is only happening with one employee.
- When they get a meeting notification the first time it comes into their Inbox like you would expect.

- If the recipient accepts the meeting invite then the next step with attachments ends up in the Deleted Items folder.

- If the meeting organizer then realizes they forgot an attachment and updates the meeting with that attachment then that Email notification goes directly into the attendees Deleted Items folder.

- If the meeting organizer updates the meeting time then everything works correctly and the attendant gets the Email update in their Inbox.

I have tried everything I can find on Google searches and nothing seems to remedy this problem so I am hoping someone has run across this or familiar with why it happens.

That smells like something client-side is “helpfully” processing meeting updates, not normal Exchange behavior. First thing I’d check is Outlook rules, including hidden/client-only rules, and whether “automatically process meeting requests and responses” or any add-in is touching calendar items. Start Outlook in safe mode with outlook.exe /safe and test with an attachment update. If it behaves, disable add-ins one at a time. Also check Deleted Items for the message properties if possible, it may show whether a rule moved it. Since it’s only one user, I’d also try a new Outlook profile before chasing server settings too hard. Outlook profiles do occasionally go feral.
