Visiting a relative in Perth Australia, my OL 2013 on my laptop keeps asking for the Hotmail password (it is MS Exchange mode as you know). However, there is NO issue with this account (the other 2 accounts, a POP3 and a Gmail IMAP are working fine), and I CAN successfully login via the "login.live" web UI access, using the same, correct password, that has not been changed, so i KNOW I have not been hacked. Never had an issue with the home ISP at all.
He is using the Telstra ISP, and I spotted an interesting post on another forum linked below, suggesting that Telstra is interfering in some way, stopping the Exchange server getting the password parsed. linking outlook to another server
Just to ask about another possible gremlin getting in the way of password authentication, I also saw this, in relation to the February posts about the infamous Microsoft shared calendar sync issues. I am not sure if this ADAL functionality is relevant or not:

2. Regedit Users Computer

Add DWORD 32: EnableADAL Hex Value:1
Close Outlook 365 reopen and there you go."
