Creating form in Outlook 2007

I am creating a form to send jobsite specific statistics. I have read the

prior posts on forms based on the message form. My problem is I do not have

access/perissions for the Organizational library. Is there another Outlook

form I can use that will readable by recipients but will work from Personal

library?

Not unless you can get every user to publish it to their own Personal Forms

library. Microsoft Infopath might be a better platform for your application.
