New Outlook 2007 form - try to send and get error Operation Failed

Greetings everyone.

I am so happy to have found this forum and hope someone may be able to help me.

I have made my first form in Outlook 2007. It is very basic, just some "labels" and "text boxes"
I have the subject line prefilled, as well as the "to" field
I performed "run this form"
I published
It is saved as an OFT
It is in my personal folder library

When I pull up the form from my library, fill in the text boxes and press "send", I get an error "Operation Failed"

I have looked high and low on the web for some clues, but no luck.

Does anyone have a suggestion or solution?

Hi Diane,
No. If I do not prefill in the "to address", the form does not give me the opportunity to type in an address.
I am lost with this. I can make a simple message template and that works, but I must be doing something wrong with this form.

I am not able to upload the file, even though I can locate it on my drive. Is there another way to post the form here?

Thank you for looking at the form.

I thought the form was pretty simple and that I followed directions, but apparently I have the ability to break the simplest of things :)

Maybe I should just go with a template, instead of a form, that may not be as easy to mess up..haha.
 
It's not published form vrs template. I don't think so, anyway - but try using a template and see if it works, because a quickie form I make in Outlook with the hidden address fields does work.
 
It worked for you?

I don't even know how to make the form without a hidden address field.
 
Thought I would "bump" this, maybe some new eyes will see and be able to offer assistance :)
