My design
My design is to create a form, and have it installed on a remote (school) network so teachers can use it. A teacher (using Outlook) selects the form as a new email and fills it out. When they press 'send", it gets sent to my computer. I can then retrieve the information from the form to enroll the teachers in our program.
Actions to enable forms
I created a custom form, not using any standard fields - all custom. Items include teacher name, grades taught, etc. I saved the form in "Personal Forms Library" with the name 'BFTApplForm', which matches what I put in the registry.
Testing
I tested by creating an email on my computer using the form, filled out the fields (garbage data), and hit 'send'. The send-to address is hard-coded and successfully sends me an email. However, the body of the email I get is blank. I tested it a dozen times and they always show up blank.
Another Bug
And then intermittently, Outlook disables my ability to edit forms. The option to "Design a Form" disappears. I have gotten it back a few times by restarting Outlook, but it's broken again. I can't edit my form.

How can I see the fields in the emailed form? and how can I tell Outlook to allow me to edit my form?

Actions to enable forms
- I have modified the registry as found in 'Registry changes to enable forms" to allow me to create and (I think) receive this one particular form via email.
- Registry looks like this: Computer\HKEY_LOCAL_MACHINE\SOFTWARE\WOW6432Node\Microsoft\Office\16.0\Outlook\Forms\TrustedFormScriptList has 2 entries, IPM.Note and IPM.Note.BFTApplForm.
- Registry also has Computer\HKEY_LOCAL_MACHINE\SOFTWARE\WOW6432Node\Microsoft\Office\16.0\Outlook\Security key with DisableCustomFormItemScript"=dword:00000000
- I customized the ribbon to turn on 'Developer" so I can get to the form options.
- I turned on TrustCenter/Email Security to allow scripts in shared folders and in public folders.
