Why is BCM causing Outlook to close?

Whenever I go to Business Contact Manager, for instance, to look up a
telephone number, then click to come back to the mail tab, Outlook closes and
I have to relaunch.

Can anyone help with this?

Turn on logging, repro the problem, and see what the log says.
